Patient Assistance Representative - Bilingual

Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

Job Description

The purpose of this role is to support identified Legacy clients with the programs under Ryan White Grant (AIDS Drug Assistance Program (ADAP), Local Pharmacy Assistance Program (LPAP), Emergency Financial Assistance (EFA), and Health Insurance Assistance, and Patient Assistance Programs (PAP) through manufacturers that provide financial assistance to the clients for medications and medical coverage. Ensures appropriate documentation is received and utilized to support the use of the funds being administered for each program. Ensures timely communication and follow up to minimize time to medication and medical coverage.

Essential Functions

  • Demonstrates understanding of various programs under Ryan White Grant in supporting patients with medications and health insurance assistance based on his/her specific role: 1) Patient Assistance Representative: ADAP, LPAP, EFA; 2) Insurance Assistance Representative: health insurance premium and cost sharing assistance (co-payments, co-insurance, deductibles, and Medicare Part D premiums)
  • Meets with clients who are eligible for the programs to obtain the appropriate documentation for enrollment
  • Demonstrates knowledge of requirement of all programs and is able to process appropriate applications or payments timely
  • Ensures a current and accurate list of people enrolled in the program is maintained
  • Performs routine and regular communication with identified client list and maintains appropriate documentation of contact
  • Completes monthly calls to ensure client engagement for maintenance of their eligibility
  • Monitors all incoming requests for appropriate and timely action to be taken
  • Documents patient care activities timely, accurately and comprehensively in all applicable electronic records, including but not limited to: Centricity, Pioneer, ARIES, Excel log as appropriate.
  • Performs timely validation of required data points to support grant standards
  • Completes month-end report reconciliation encounters are reported accurately
  • Identifies concerns and issues and shares with appropriate service providers supporting the client using the appropriate documentation processes established.
  • Performs reviews and audits as assigned to support accurate reporting on a regular and routine basis
  • Collaborate and maintains good working relationship with peers and other colleagues and those providing care to the patient
  • Special projects as needed.
  • Performs other duties as assigned.

Education & Training Requirements

  • High school diploma required
  • Minimum (1) year of healthcare experience or,
  • Minimum (3) years of customer service experience, preferably in healthcare

Work Experience Requirements

  • Knowledge of healthcare terms, HIV disease and medications to treat HIV and associated illness desired
  • Insurance and/or COBRA knowledge
  • Proficient written and verbal communication skills
  • Demonstrates knowledge of MS Excel and MS Word
  • Bilingual-Spanish preferred
  • Ability to meet Legacy Community Health’s EPIC proficiency requirements.
