Pharmacist – Medication Therapy Management FT

Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

Job Description

Pharmacist – Medication Therapy Management provides medication therapy management to specialty pharmacy patients, including enrollment and reassessments, patient counseling, and drug utilization reviews to optimize therapy.

Essential Duties and Functions

  • Provides MTM service to Specialty pharmacy patients.
    • Promotes and enrolls patients into Specialty Pharmacy program.
    • Performs 6 month and annual reassessment for all patients.
    • Performs drug utilization reviews to prevent duplications, drug-drug interactions, and to optimize therapy.
  • Performs drug utilization reviews in Outcomes MTM for Medicare patients to ensure the patient has optimal medication therapy and to increase medication adherence.
  • Bridges gaps in care through patient outreach, medication profile reviews, and recommendation of therapy changes.
  • Provides counseling and education to the patients as appropriate.
  • Assesses medication adherence during the process medication counseling.
  • Ensures the appropriateness of all medication orders, new prescription or refills, in reference to the patient’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of the active medication profile, provider notes, and available pertinent laboratory information.
  • Practices pharmacy in compliance with all governing regulatory authorities.
  • Review patient profile to ensure appropriate drug selection and dosage.
  • Documents adverse drug reactions and all relevant interactions with patient and healthcare provider.
  • Promotes the practice of evidence-based medicine by keeping abreast of new drugs, devices, and medical literature.
  • Provides accurate, adequate drug information/education to patients, families, and providers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
  • Demonstrates the ability to set priorities, work independently and cooperatively with others and within the department.
  • Demonstrates competence related to financial reimbursement from available grants, prescription assistance programs and understands 340B requirements for eligible patients, replenishing inventory and special billing conditions.
  • Presents information (verbal and/or written) in a clear and concise manner.
  • Displays friendliness, compassion, sensitivity, kindness, and appropriate manners in interactions with patients, providers, and other ancillary healthcare staff.
  • Displays open-mindedness and objectivity in relations with patients, providers, and other ancillary healthcare staff.
  • Promotes a spirit of unity which enables the group to work together to achieve a common goal.
  • Takes initiative to perform job and assist coworkers, supporting the work of others in accomplishing the mission of Legacy Community Health.
  • Displays adaptability by adjusting to meet the needs of the patient and organization.
  • Participates in the training/education of employees, pharmacy students, and other healthcare professionals as documented on attendance form or through evaluations completed by the participant.
  • Incorporates constructive feedback in a positive manner for improvement and necessary changes in, but not limited to, scheduling, procedures, and assignments.
  • Exhibits behaviors which demonstrate competence, reliability, self-control, honesty, and appropriate overall appearance while rendering service.
  • Checks all means of communication including email at least once per shift.
  • Keeps immediate supervisor informed of issues.
  • Performs other duties as assigned.

Education, Certification & Training Requirements

  • Graduate of an accredited college/university of pharmacy (Pharm.D. or B.S. with equivalent experience).
  • Current Texas Pharmacist licensure in good standing (or eligible).
  • Certification in immunization delivery, smoking cessation, and naloxone (or eligible)
  • Computer experience, including Windows, Excel, and accounting software.
  • Must maintain First Aid/CPR.
  • Spanish speaking preferred.

Required/ Preferred Work Experience

  • 2+ years of healthcare experience is required.
  • Post-graduate residency preferred.
  • 1+ years of community pharmacy experience preferred.
  • 1+ years of medication adherence counseling experience preferred.
  • 1+ years of Medicare market experience preferred.
  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics preferred.

Required Knowledge, Skills and Abilities

  • Pleasant, professional, and articulate phone voice
  • Ability to multi-task and handle many incoming calls at once
  • Efficient and accurate data-entry skills
  • Demonstrates strong computer skills.
  • Bilingual in English/Spanish preferred.
