Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

Job Description

The Director of Social Services manages the daily activities and provides direct supervision of Clinic Social Worker Manager and Clinic Social Work Supervisors. The Director of Social Services with a LCSW can maintain a therapy caseload to provide therapeutic services to patients one day per week. The Director of Social Services will monitor quality assurance, engage with regional leadership, develop agency and program specific trainings, and participate in agency-wide/program initiatives.

Essential Functions.

  • Providing overall management of the daily activities of the Social Services Department.
  • Providing direct supervision to all members of the Social Services, which includes any social workers, chemical dependency counselors, social work associates, and administrative staff.
  • Provide direct supervision to all volunteers in the Social Services Department.
  • Overseeing all aspects of the grant management for the Social Services Department, which includes grant submission, monthly reporting, and quarterly reporting.
  • Provide coverage for staff when out due to absenteeism.
  • Provide direct client care as needed.
  • Document client notes in social services and medical files as appropriate.
  • Conduct weekly meetings with social service staff for problem resolution, case discussions, and coordination of care.
  • Conduct evaluation of social services programs as indicated by Quality Assurance and grant contracts
  • LMSW must obtain their LCSW by the standard set forth by the Texas State Board of Social Work Examiners within two and one half (2.5) years of beginning the process.
  • Participates in the Performance Improvement Program
  • Interviews and advises patients regarding health and illness prevention, including FQHC-required voluntary family planning services and related preventative health services in compliance with funding requirements and state and federal laws, rules and regulations.
  • Performs other duties as assigned.

Education & Training Requirements:

  • Must be a Licensed Master of Social Work.
  • Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers’ compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy.

Work Experience Requirements

  • Has at least 3-5 years of management/supervision experience in therapy or social work services
  • Experience with overseeing large-scale projects
  • Experience working within a FQHC setting
  • Familiarity working with Texas Children Pregnant Women (CPW) program
  • Have strong leadership, organizational, interpersonal & communication skills
  • Must be able to manage multiple deadline and decision making responsibilities. (Mgr. Only)
  • Adept in decision making responsibilities
